You authorize PBM to broadcast images of the concert on PBM's website.^*

IT IS FORBIDDEN TO PLACE OBJECTS ON THE NEW PIANOS, MOVE CHAIRS BEFORE OR AFTER THE CONCERT. NO FOOD OR SUGARY DRINKS ALLOWED.

Upon receipt of this form, we will send you an agreement to sign and return at least 5 days before the event.
